Prediction and control of complex cardiac rhythms in the heart
Sudden cardiac death (SCD) is a major public health problem and is one of the leading causes of mortality in the Western world. SCD is caused primarily by ventricular fibrillation (VF). Clinical studies showed that T-wave alternans, defined as an alternating change in the amplitude or shape of the T-wave in the ECG, is a precursor of cardiac electrical instability and, consequently, SCD. It has also been demonstrated that action potential duration (APD) alternans at the single-cell level is a primary cause of T-wave alternans. Since alternans is a precursor of SCD, prediction of alternans is a crucial step in detecting and predicting fatal arrhythmias.
In the past years, advances have been made to predict APD alternans on the cellular and heart levels. Specifically, two main criteria for APD alternans prediction were developed: restitution-based criterion and data-driven approach. These two approaches are distinct but complementary, and target at different aspects of cardiac dynamics. They have been mostly verified using single-cell numerical simulations and experiments. The direct relationship between APD alternans in the heart and T-wave alternans in the ECG in terms of prediction is still missing. There two major goals in the projects.
1. To provide experimental validation of the two approaches of APD alternans prediction in the spatially extended system, i.e. heart.
2. To translate the two approaches for APD alternans prediction into framework that can predict alternans based on non-invasive ECG recordings.
